Responsibilities
  • Provide physical therapy services to clients in their homes, including assessments, goal setting, and treatment planning.
  • Develop and implement individualized care plans to meet the unique needs of each client.
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordination.
  • Communicate effectively with clients, families, and healthcare teams to ensure seamless care and support.
  • Document client progress and outcomes to ensure accurate and timely reporting.
Requirements
  • Current licensure as a Physical Therapist in the state of New Jersey.
  • Minimum of one year of experience as a Physical Therapist in a home health care setting.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work a variety of schedules, including evenings and weekends.
