We are seeking a dedicated Physical Therapist to join our team in Hornell, NY. This role offers a unique opportunity to make a significant impact on residents' lives in a skilled nursing setting while enjoying the beauty of New York's Finger Lakes region.
Key Responsibilities:- Conduct comprehensive evaluations of residents' conditions upon admission
- Develop and implement personalized physical rehabilitation plans
- Utilize various techniques including exercise therapy, manual therapy, and physical therapy devices
- Improve residents' mobility and pain management
- Educate residents on proper use of physical therapy equipment
- Monitor and document residents' progress throughout their recovery
- Collaborate with interdisciplinary teams to ensure holistic resident care
- Supervise physical therapy assistants as needed
- Maintain a safe, clean, and confidential working environment
- Adhere to infection control policies and all legal/professional requirements
- Graduate of an accredited Physical Therapy Program
- Current New York State PT License
- Experience in skilled nursing or long-term care (preferred but not required)
- Strong communication and interpersonal skills
- Commitment to resident-centered care and continuous professional development
