At RVNAhealth, we are seeking a skilled and compassionate Home Health Aide Supervisor to join our team. As a key member of our StayingWELL team, you will provide assessment visits in clients' homes, develop personalized care plans, and supervise Aides as needed.

Responsibilities:
  • Meet with new clients to review agreements and establish care plans
  • Review and update care plans as needed, ensuring clients receive the best possible care
  • Determine and communicate with Client Services Coordinators the skills/criteria necessary for a positive Client/Caregiver match
  • Communicate changes in care plans/client needs with clients and/or family members
  • Resolve complaints made by patients/families regarding care or services received
  • Identify and manage high-risk clients, referring to home health and hospice as needed
  • Provide exceptional customer service to clients and caregiver staff
  • Actively develop and pursue referral sources, facilitating ongoing relationships with ALFs
  • Cover for other HHA Supervisors in their absence
Qualifications:
  • Current license in a relevant field (RN/LPN/OT/PT) in the State of Connecticut, or a Bachelor's degree in Nursing, Healthcare, or a related field with 2+ years of experience as a CNA
  • Minimum of 2 years of experience in home care or a related field
  • Management experience
  • Reliable transportation
  • Organized and detail-oriented
  • Ability to communicate clearly with patients, families, visitors, leadership, and others
