As a Physical Therapist, you will be responsible for developing and implementing individualized Physical Therapy plans of care for our residents and short-term patients. You will work closely with our interdisciplinary team to ensure seamless care delivery and excellent patient outcomes.

Key Responsibilities:

  • Develop and implement Physical Therapy plans of care for residents and short-term patients.
  • Supervise Physical Therapy Assistants as needed to ensure delivery of high-quality care.
  • Conduct patient screenings to identify the need for Physical Therapy services.
  • Communicate progress and barriers to interdisciplinary team members.
  • Complete timely and accurate Physical Therapy documentation in compliance with regulatory requirements.
  • Maintain a safe and clean treatment area, adhering to infection prevention protocols.
  • Provide patient, family, and caregiver education on PT plans of care and recommendations for follow-up care.

Requirements:
Current Texas licensure as a Physical Therapist required. New graduates welcome to apply.

Work Environment:
Long-term care, nursing home, and rehabilitation center settings.
