Nursing Care Coordinator for Long-term Care

Job Overview:

We are seeking a skilled Registered Nurse to join our team as a Minimum Data Set (MDS) Coordinator at Genesis HealthCare. This role plays a critical part in ensuring appropriate reimbursement for patient care services by managing the Patient Assessment Process.

In this position, you will develop individualized care plans, coordinate assessments, and train staff on Activities of Daily Living. Our supportive environment offers ongoing education and training for career advancement in healthcare services.

Salary:

The estimated salary for this position is $42-$45 per hour, depending on experience.

Responsibilities:
  • Develop individualized patient care plans, identifying patient needs and projected outcomes as required by Federal and State regulations.
  • Contribute to the nursing center interdisciplinary Resident Assessment Instrument (RAI) process while following RAI guidelines for completion, computer encoding and electronic transmission of the MDS.
  • Assist with Activities of Daily Living training for new hires and through ongoing continuing education to existing staff.
Requirements:
  • A current Registered Nurse license in good standing.
  • Previous experience in a hospital, long-term facility, or other healthcare-related facility.
  • Prior experience completing MDSs and Care Plans.
