We are seeking a skilled Pharmacy Liaison to join our team at Optum, a leading health services company. As a Pharmacy Liaison, you will play a critical role in providing high-level, specialized prescriptive care to patients in a fast-paced hospital and/or specialty clinic setting.
Key Responsibilities:- Establish and maintain solid relationships with patients, hospital staff, and pharmacy team members.
- Participate in daily/weekly/monthly team meetings and act on feedback to ensure quality care on-site.
- Educate patients about services provided at time of discharge and articulate how patients may utilize pharmacy services to obtain full benefits of high-quality prescriptive care.
- Attend and contribute to daily medical team meetings and coordinate the collection of prescriptions, patient orders, and all necessary paperwork for the pharmacy team.
- Schedule and coordinate pharmacist consultation review of patient's prescribed medications to drive medication adherence and education.
- Handle the coordination of patient medication delivery at time of discharge and ensure education has been provided to all necessary caregivers.
- Identify and resolve insurance billing adjudication, including facilitating prior authorizations and assisting in locating financial assistance for patients as needed.
- Conduct weekly follow-up calls with hospital patients following discharge to ensure aftercare plans have been met.
- Identify trends, barriers, and potential process improvement opportunities and recommend customized and innovative solutions.
- National Pharmacy Technician Certification
- Pharmacy Tech License for the state of TN
- Valid driver's license and insurance
- Access to reliable transportation
- 2+ years of hospital experience or pharmacy technician experience
- Experience in a behavioral or mental health setting
- Highly proficient in Microsoft Office, particularly Excel and Word
- Proven solid verbal and written communication skills
- Proven ability to work with a close-knit team and possess solid administrative and organizational skills
