Key Responsibilities- Perform annual comprehensive health assessments in the home
- Work with a team to deliver exceptional personalized care to patients
- Travel up to 120 miles per work day, with mileage reimbursement
- Collaborate with the Contact Center to build your schedule based on your availability
- Board Certified Family Nurse Practitioner, Acute Care Nurse Practitioner, or Adult-Geriatric Nurse Practitioner
- Active OH state license required
- Experience in urgent care or ambulatory care a plus
- Compensation: $90 per completed assessment
- 401K and limited voluntary benefits
- Mileage reimbursement plan
