We are seeking a highly skilled Registered Nurse to join our team at SaVida Health, a leading provider of outpatient opiate and alcohol addiction treatment services. As a Spoke Nurse, you will play a critical role in delivering comprehensive care management, care coordination, and health promotion services to patients with Substance Use Disorder.
Key Responsibilities- Comprehensive Care Management: Develop and implement individual plans of care for patients, working closely with the site team to identify patients for Medication Assisted Therapy and conduct initial assessments.
- Care Coordination: Coordinate services and supports across treatment and human services settings, ensuring seamless transitions of care and reducing hospital readmissions.
- Health Promotion: Promote patient activation and empowerment, supporting healthy behaviors and self-management of health, mental health, and substance abuse conditions.
- Comprehensive Transitional Care: Streamline the movement of patients from one treatment setting to another, facilitating timely development of community placements and coordinating sharing of necessary treatment information among providers.
- Individual and Family Support: Assist patients in fully participating in treatment, reducing barriers to accessing care, and promoting recovery.
- Referral to Community and Social Support Services: Assist patients in obtaining and maintaining eligibility for formal supports and entitlements, and facilitate connection to informal community resources.
- Current RN licensure in Vermont required, BSN preferred.
- Demonstrated ability to connect patients to area resources, serve as a patient advocate, and effectively perform case management.
- Excellent interpersonal skills, ability to effectively deal with change, and capability of interfacing with the community in an organized and collaborative way.
- Ability to work independently and as part of a team, plan and prioritize work with limited supervision.
- Working knowledge of area social services, justice, health, community service, and governmental agencies a plus.
- Ability to serve as a liaison to participants, monitors, employees, and professional liability insurance providers and provide advocacy as needed.
- One-year experience using an Electronic Health Record, previous use of Athena Electronic Health Record (or EMR) a plus.
- Computer literacy, typing skills, knowledge of EHR/EMRs.
- Ability to work with individuals and groups from diverse populations, bilingual/bicultural preferred.
- Excellent verbal and written communication skills, ability to present information in a group setting.
- Ability to effectively manage time to meet deadlines, administer medical injections, and proficiency in clinical documentation.
