About the RoleNurse Practitioner - In-Home Care
As a Nurse Practitioner at EasyHealth, you will be at the forefront of delivering preventive care and educational services, directly engaging with members in the heart of their communities.
This position offers the unique fulfillment of local travel and the opportunity of face-to-face interactions with members directly in the comfort of their homes.
Key Responsibilities- Perform comprehensive assessments such as Annual Wellness & Quality Gap Care visits for our members in their homes.
- Accurately code, verify, and document clinical findings.
- Complete a detailed evaluation of the medical and social assessment of the beneficiary members using the company's proprietary tool or client EMR.
- Identify and refer members/patients for routine, urgent, and emergent case management.
- Make recommendations for member's follow-up with their primary care physician.
- 2 years of hands-on experience as a Nurse Practitioner.
- Current, unrestricted license in the state(s) in which the services are rendered.
- Board-certified, preferably geriatric or adult medicine specialty.
- Familiarity with the HCC model and quality HEDIS and other STAR measures.
- Effectively communicate with elderly and chronically ill members.
- Proficient in the use of computer, tablet, and mobile technology.
- Willingness to travel within the geographic area.
