As a Registered Nurse in our Home Health Visiting Nurse program, you will provide comprehensive skilled nursing care and services to patients in their homes. You will work collaboratively with an interdisciplinary team to develop and implement individualized care plans that address the unique needs of each patient.
Key Responsibilities- Conduct thorough assessments of patients to identify their needs and develop effective care plans
- Provide skilled nursing interventions, including wound care, medication management, and education on disease management and self-care
- Collaborate with patients, families, and healthcare providers to ensure seamless care transitions and optimal patient outcomes
- Document patient information accurately and timely in the electronic medical record
- Participate in quality improvement initiatives to enhance patient care and safety
- Current RN license in the state of Washington
- Minimum 2 years of experience as a Registered Nurse in medical-surgical, home health, or hospice settings
- Associates degree in Nursing from an accredited program
- Basic Life Support certification
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
- Experience with home care systems, such as Horizon
- Bachelor's degree in Nursing or a related healthcare field
