Position Overview:
PeaceHealth is in search of a dedicated Pharmacist for a Full Time, 1.00 FTE, Day position.

Key Responsibilities:
This role is pivotal in ensuring the effective use of medications to enhance patient outcomes and quality of life. Responsibilities include:

  • Reviewing medication orders for accuracy and appropriateness, including contraindications, allergies, dosage, and frequency.
  • Clarifying and rectifying physician medication and IV orders, including the medication reconciliation process.
  • Selecting medications for outpatient dispensing and interpreting prescriptions.
  • Monitoring patients on specific target drugs and initiating pharmacokinetic dosing protocols.
  • Evaluating drug therapy for appropriateness and making recommendations to clinicians.
  • Prescribing drug therapy in accordance with collaborative management agreements.
  • Dispensing medications and providing patient counseling.
  • Detecting, monitoring, and reporting adverse drug events and medication errors.
  • Supervising and training pharmacy technicians and interns to ensure accurate order entry and preparation.
  • Participating in the development of drug protocols and clinical pathways.
  • Conducting drug usage evaluations and reporting findings to the Pharmacy & Therapeutics Committee.

Qualifications:
  • Bachelor's Degree in Pharmacy or Doctor of Pharmacy from an accredited institution.
  • Required: Demonstrated clinical pharmacy skills and customer service abilities.
  • Required: Current licensure as a Pharmacist in Alaska, Oregon, or Washington, or eligibility to obtain such licensure.
  • Preferred: Basic Life Support, Advanced Cardiac Life Support, and Pediatric Advanced Life Support certifications.

Skills:
  • Proficient in basic computer skills and MS Office applications.
  • Ability to identify medications and read graduation marks on syringes and flasks.
  • Strong organizational, planning, and prioritization skills.
  • Excellent verbal and written communication skills.
  • Ability to think independently and act decisively.
  • Strong mathematical and analytical abilities.

Working Conditions:
  • Ability to move around the work area with occasional sitting.
  • Occasional squatting, kneeling, and bending.
  • Fine motor skills for precise procedures.
  • Frequent reaching and occasional lifting of up to 30 lbs.
  • Exposure to biohazards, body fluids, and airborne particles.
