We are seeking a highly skilled Senior Pharmacist to join our Specialty Pharmacy team at Community Health Network. As a key member of our team, you will provide clinical and operational support for our ambulatory clinics, ensuring the highest level of care for our patients.

Key Responsibilities
  • Provide clinical support for the initiation and monitoring of specialty medication therapy for self-administered medications.
  • Develop and maintain clinical pharmacy pathways for the treatment of complex conditions, including Hepatitis C, Cancer, Cystic Fibrosis, HIV, Bariatrics, and Rheumatology.
  • Collaborate with healthcare teams to ensure seamless patient care and optimal medication therapy outcomes.
  • Stay up-to-date with the latest clinical guidelines and research to provide evidence-based care.
Requirements
  • Active and valid Indiana Pharmacist license.
  • 4-5 years of experience in a specialty, ambulatory, hospital-based, or closed-door long-term care pharmacy.
  • Doctorate degree preferred.
