We are currently seeking a Clinical Nurse Specialist to join our team at Teleios Collaborative Network. This role involves supporting the admission of new patients into hospice care, ensuring a smooth transition and comprehensive initial care planning.

Key Responsibilities

  • Engage with patients and their families to explain hospice services and philosophy, while securing consent for care.
  • Formulate an initial care strategy that identifies immediate objectives and interventions, grounded in nursing assessments.
  • Collaborate with hospice personnel and other healthcare facilities as necessary to ensure optimal patient care and resource utilization.
  • Deliver direct nursing and supportive services to patients facing terminal illnesses.

Work Schedule

  • Three 12-hour shifts on weekends, with hours from 9 AM to 9 PM or 10 AM to 10 PM.

Qualifications

  • Current Registered Nurse License.
  • BLS Certification.
  • Valid Driver's License.
