Job Title: Case Manager

We are seeking a highly motivated and empathetic Case Manager to join our team at the YMCA of Greater Erie. As a Case Manager, you will play a critical role in supporting individuals transitioning back into the community after incarceration.

About the Role

The successful candidate will be responsible for developing and coordinating service support plans for transitioning clients and community-based clients. This will involve working one-on-one with individuals to identify their needs and connecting them with appropriate services to eliminate barriers to follow-through.

You will also be responsible for coordinating linkages and referrals to community providers for employment opportunities, education/training, housing, transportation, treatment services, mentoring, and other comprehensive supports and services.

Responsibilities
  • Develop and coordinate service support plans for transitioning clients and community-based clients
  • Work one-on-one with individuals to identify their needs and connect them with appropriate services
  • Coordinate linkages and referrals to community providers for employment opportunities, education/training, housing, transportation, treatment services, mentoring, and other comprehensive supports and services
  • Build relationships with the community and leverage those contacts to support the program
  • Keep accurate and up-to-date records and ensure compliance with YMCA standards and government regulations
Requirements
  • Bachelor's Degree in Criminal Justice, Public Administration, Organizational Leadership, or related field
  • At least 2 years of experience in restorative justice, victim advocacy, offender services, or related field
  • Experience in supporting, empowering, and managing individuals
  • Excellent oral, auditory, and written communication skills
  • Ability to work with a diverse group, including the prison population
  • Flexibility and ability to adapt to changing circumstances
  • Working knowledge of computer systems
  • Ability to adhere to confidentiality and security policies
  • Valid PA driver's license and full-time access to reliable, private transportation
  • PA Child Abuse, PA State Police Criminal Records Check, and other applicable clearances and required pre-employment trainings
