About the Role

Accompany Health is seeking a highly skilled Clinical Pharmacist to join our Integrated Care Teams. As a key member of our team, you will play a vital role in providing and coordinating holistic, patient-centered care for individuals with complex medical, behavioral health, and social needs.

Responsibilities
  • Ensure evidence-based, safe, and cost-effective drug therapy for patients in collaboration with the Integrated Care Team and external providers.
  • Provide direct and consultative care to patients and their caregivers in a Comprehensive Medication Management (CMM) model to identify and resolve medication therapy problems.
  • Design individualized therapeutic regimens for optimal management of chronic disease states, such as diabetes, CHF, COPD, and CKD/ESRD.
  • Lead processes to ensure appropriate access, affordability, and adherence to medication regimens.
  • Support efforts to improve HEDIS medication adherence and other pharmacy quality measures.
  • Collaborate with cross-functional teams to promote adherence to prescribed therapies.
  • Provide day-to-day oversight of pharmacy program workflows to support the pharmacy team in implementing targeted interventions to improve adherence rates.
Requirements
  • Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy.
  • Active pharmacist license in the state of Michigan, Indiana, or Colorado.
  • Minimum of 2 years of experience in HEDIS-related medication adherence work, quality improvement, population health, or collaborative care with a multidisciplinary team.
  • Foundational knowledge of HEDIS measures, particularly those related to medication adherence.
  • Proficiency in using EHR systems and data analysis tools.
  • Excellent communication, problem-solving, and organizational skills.
  • Ability to work independently as well as in a team-oriented environment.
Preferred Qualifications
  • Active Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP), Geriatric Pharmacy (BCGP), or a relevant clinical area OR certification in Medication Therapy Management (MTM).
  • Understanding of value-based care models and their impact on medication adherence.
  • Experience in a managed care setting or a healthcare quality improvement role.
  • Experience in managing complex patients with hypertension, hyperlipidemia, diabetes, heart failure, or COPD in a multidisciplinary model.
  • Experience with managing behavioral health medications, including medication-assisted treatment.
  • Experience with motivational interviewing or other patient engagement techniques.
What We Offer

A competitive salary range of $120,000 - $150,000 per year, plus bonus, equity, and benefits. Our salary ranges are determined by role, level, and location. We also offer a comprehensive benefits package, including medical, dental, and vision insurance, 401(k) matching, and paid time off.
