We are seeking a compassionate and skilled Private Duty Nurse LPN to join our team at Aveanna Healthcare. As a Private Duty Nurse, you will provide one-on-one care to our patients in their homes, helping them to recover from hospitalization and maintain their independence.
Key Responsibilities- Implement the physician's prescribed plan of care, ensuring the highest level of patient satisfaction and well-being.
- Conduct patient assessments and coordinate care with other healthcare professionals to ensure seamless transitions.
- Administer prescribed medication, treatments, and therapies, maintaining accurate and timely documentation in our system of record.
- Provide health education and training to patients and their families, empowering them to take an active role in their care.
- Collaborate with the care team to address patient concerns, changes in condition, and other issues that may arise.
- Active, unencumbered license (RN/LPN/LVN) in the state where you will practice.
- Current CPR certification from a recognized organization.
- TB skin test (current within the last 12 months).
- At least six months of prior hands-on nursing experience, although this is not required.
- Reliable transportation to and from patient homes.
