Tact Staff is seeking a dedicated travel nurse RN specializing in Case Management for an exciting opportunity in the field of Acute Care Case Management.
Job Details:

Key Responsibilities:
The selected candidate will be responsible for:
- Coordinating care and planning transitions throughout the acute care patient journey.
- Collaborating with physicians, utilization managers, and medical social workers to ensure timely patient progression and appropriate care transitions.
- Facilitating effective communication and resource coordination across the continuum of care.
- Engaging with patients, families, and interdisciplinary teams to optimize health outcomes and resource utilization.
- Addressing complex clinical and social situations, particularly in emergency settings, to prevent unnecessary admissions.
Qualifications:
To be considered for this role, candidates must possess:
- Valid CA RN Licensure
- A minimum of 2 years of experience in Case Management
- BLS certification (AHA)
- Experience in Utilization Management with attending physicians
