Tenet Massachusetts is seeking a highly skilled Social Work Lead to join our team in Natick, Massachusetts.

Job Summary

The Social Work Lead will be responsible for facilitating care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources. This role will integrate national standards for case management scope of services, including transition management, care coordination, compliance with regulatory requirements, education, and leadership.

Key Responsibilities
  • Conduct complex psycho-social assessments and interventions to promote timely throughput, safe discharge, and prevent avoidable readmissions.
  • Coordinate complex discharge planning and educate patients, families, and caregivers on post-discharge care.
  • Lead a population of patients by service line and/or lead the team, providing guidance and support to social work staff.
  • Act as a resource to all staff members regarding policies and procedures, as well as community resources.
  • Collaborate with the Director of Case Management and the team to trend barriers to care and resolve them.
Requirements
  • Master's of Social Work (MSW) degree.
  • 3 years of department experience or 4 years of acute hospital social work experience.
  • LCSW or LMSW licensure, with active license for the state of practice.
  • Preferred: Accredited Case Manager (ACM) certification.
