Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Sullivan County Health Care in Claremont, NH. As an LPN, you will play a vital role in providing high-quality care to our residents, working closely with our interdisciplinary team to ensure the best possible outcomes.

Responsibilities
  • Provide direct patient care, including administering medications, performing wound care, and assisting with daily living activities
  • Conduct thorough assessments of residents' physical and emotional needs, developing and implementing individualized care plans to meet those needs
  • Collaborate with the interdisciplinary team to coordinate care, including physician orders, therapy services, and social services
  • Supervise and delegate tasks to nursing assistants, ensuring that care is delivered in a safe and efficient manner
  • Participate in quality improvement initiatives, contributing to the development and implementation of policies and procedures to enhance resident care and safety
  • Communicate effectively with residents, families, and healthcare team members, ensuring that all parties are informed and engaged in the care process
  • Document resident care and progress accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines
  • Assist with the orientation of new nursing staff, providing guidance and support to ensure a smooth transition
  • Participate in ongoing education and training, staying current with best practices and regulatory requirements
Requirements
  • Current LPN licensure in New Hampshire or compact state
  • 3+ years of experience in long-term care, with a focus on gerontology and rehabilitation
  • IV certification preferred
  • Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with diverse teams
  • Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions
  • Ability to maintain confidentiality and adhere to HIPAA guidelines
  • Willingness to work a night shift, with flexibility to work varied schedules as needed
