Case Manager

We are seeking a highly skilled and compassionate Case Manager to join our team at First Steps Recovery. As a Case Manager, you will play a vital role in coordinating and providing individualized counseling and clinical services to clients at our inpatient and outpatient levels of care.
Key Responsibilities- Develop and implement comprehensive treatment plans, including prioritized problem lists, client-reported goals, and measurable objectives.
- Provide individual and group counseling sessions, utilizing evidence-based practices and adhering to First Steps Recovery standards and policies.
- Document client progress, prepare effective level of care assessments, and develop strong discharge plans and summaries.
- Collaborate with the treatment team to plan and coordinate client care, ensuring seamless transitions and optimal outcomes.
- Ensure compliance with CLIA standards, protocols, and procedures for urinalysis collection, documentation, and disposal.
- Participate in weekly treatment team meetings, staff meetings, and in-service training to stay current on best practices and agency policies.
- AA or Bachelor's degree in Human Services or Behavioral Health, or equivalent experience.
- Drug and Alcohol Counseling Certification or Registration (CCAPP, CAADE, CADTP, or Affiliate).
- 1-2 years of experience in addiction and mental health or equivalent human services work experience.
- Proficiency in KIPU Systems Resident Management Software and Microsoft Office.
