Responsibilities- Supervise day-to-day clinical and office operations, ensuring compliance with professional standards, agency policy and procedure, and state and federal regulatory requirements.
- Coordinate referrals and patient care with an interdisciplinary team approach, ensuring the development, implementation, and updating of individualized plans of care.
- Evaluate the quality of clinical care, ensuring that patient needs are continually assessed, and collaborate with clinicians and physicians regarding patient transfers or discharges.
- Manage KPIs, workflow, escalation/complaint calls, and guide professional clinicians on implementing Emergency Care Protocols.
- Assure clinicians, including contract staff, have required training, competency, and supervision, and make patient and personnel assignments, ensuring that assigned staff have demonstrated competence in required tasks/services.
- Participate in QA and performance improvement activities, review and investigate incidents, analyze for trends, and implement corrective action plans as needed.
- Registered nurse with current licensure to practice nursing in the practicing state.
- Possess and maintain CPR certification.
- Previous experience in home care setting with two years management or supervisory experience, preferred.
- Proven ability to work within an interdisciplinary setting.
- Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and is in good working order.
- Prior use of Electronic Medical Records (EMR) preferred.
