Key Responsibilities- Conduct regular wellness visits for all residents, monitoring their health and well-being.
- Develop and implement individualized care plans, working closely with residents, families, and healthcare teams.
- Collaborate with the Resident Care Director to ensure seamless care delivery, addressing any concerns or issues promptly.
- Provide clinical support and guidance to community team members, promoting a culture of excellence and resident-centered care.
- Stay up-to-date with regulatory requirements, ensuring compliance with federal, state, and local regulations.
- Participate in quality improvement initiatives, contributing to the development of best practices and policies.
- Graduate of an approved college or school of nursing.
- Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
- Minimum of one (1) year experience in home health, assisted living, hospital, or long-term care environment.
- Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare teams.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
- Proficiency in computer skills, Microsoft Office, and Sunrise applications.
