Job Overview

Position Summary:

Deliver exceptional nursing care to adolescents under the guidance of ACS and OCFS within a residential environment. Oversee healthcare coordination and medical case management for clients. Conduct essential health evaluations, including monitoring vital signs, blood pressure, and weight. Provide continuous health supervision to address and support the health requirements of clients. Make suitable referrals and ensure follow-up for medical, substance use, and psychiatric treatments. Observe, track, and document client medication self-management. Collaborate with healthcare and psychiatric professionals as well as hospitals. Design and lead health-focused educational sessions and individual client instruction. Engage in interdisciplinary team discussions and case reviews. Act promptly in medical emergencies, providing first aid/CPR and Narcan for opioid overdose reversal when necessary. Address other program-related crises as required. Ensure adherence to regulatory and organizational standards. Additional responsibilities may be assigned. Must be willing to travel to various sites.

Qualifications:

  • Graduated from an Accredited Licensed Practical Nursing Program and holds a valid New York license
  • Possesses a valid NYS Driver's License in good standing
  • Experience working with children
  • Strong communication abilities
  • Exhibits maturity, calmness, and adaptability
  • CPR Certification required
  • First Aid Certification required
  • At least 2 years of experience as an LPN; experience in pediatric, psychiatric emergency, or residential treatment is preferred
