Key Responsibilities- Conduct comprehensive patient screenings and assessments to identify nutritional risks and develop personalized nutrition plans.
- Collaborate with healthcare teams to ensure seamless care coordination and effective communication.
- Implement targeted nutrition interventions and provide education to patients and families on healthy eating habits and disease management.
- Monitor and evaluate patient progress, making adjustments to care plans as needed.
- Participate in quality improvement initiatives and contribute to the development of evidence-based nutrition practices.
- Two years of acute hospital experience working with patients in complex care settings.
- Bachelor's degree in Nutrition or related field with completion of an accredited dietitian program.
- Valid registration as a Registered Dietitian with the Commission on Dietetic Registration (CDR).
- Current ServSafe Allergen and Food Handler Certifications.
