**Job Summary**

PRN Healthcare Services is seeking a skilled Travel Nurse RN Case Manager to join our team. As a Travel Nurse RN Case Manager, you will be responsible for providing high-quality patient care and case management services to patients in a variety of settings.

**Key Responsibilities:**

  • Provide patient care and case management services to patients in a variety of settings, including hospitals, clinics, and long-term care facilities.
  • Assess patient needs and develop individualized care plans to meet those needs.
  • Collaborate with healthcare teams to ensure seamless patient care and transitions.
  • Document patient progress and outcomes in a timely and accurate manner.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.

**Requirements:**

  • Current RN licensure in the state of practice.
  • Minimum of 1 year of experience as a Registered Nurse in a case management role.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Basic computer skills and proficiency in electronic medical records.
