We are seeking a Nurse Practitioner to join our HouseCalls team in Bonner County, Idaho. As part of our integrated medical teams, you will provide annual clinical assessments to patients in their homes, helping identify and reduce health risks while coordinating follow-up care.

You will have the opportunity to achieve work-life balance with flexible scheduling, no on-call requirements, and no weekends or holidays.

Key Responsibilities:

  • Conduct in-home assessments, including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams
  • Perform evidence-based practice screenings, including point-of-care testing as appropriate
  • Identify diagnoses for care management and active medical management in furtherance of treatment
  • Formulate lists of current and past medical conditions using clinical knowledge and judgment and assessment findings
  • Communicate assessment findings to inform members' primary care physicians of potential gaps in care
  • Identify urgent and emergent situations and intervene appropriately
  • Educate members on topics such as disease process, medication, and compliance

Requirements:

  • Active NP license or ability to obtain by start date, unencumbered and free of any open/unresolved disciplinary actions
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or ability to obtain national certification and/or NP license in state of assignment by start date
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag in and out of the car, navigating stairs, and various dwelling conditions, and sitting, standing, and kneeling for physical assessments
  • Reliable transportation for home visit assessments

Estimated Salary Range: $115,000 - $140,000 per year, depending on experience. This estimate is based on industry standards and regional factors.
