Job Summary:

The Weekend Hospice RN is a professional nurse who provides on-call services for the personal, physical, emotional, and social needs of patients and their families. This role requires a strong commitment to delivering high-quality care and a passion for working in a dynamic and supportive environment.

Responsibilities:
  • Evaluate and respond to patient needs via telephone contact, home visits, and/or facility visits
  • Develop and maintain a written Hospice Care Plan for each patient and update as needed
  • Maintain accurate and up-to-date medical records and other records pertaining to patient care
  • Participate in weekly patient care conferences and communicate with team members regarding patient status
  • Collaborate with Interdisciplinary Group members to ensure the best care for patients and their families
  • Develop Hospice Aide plans of care based on patient personal care needs
  • Notify Team Leader of changes in patient status, including hospitalizations and discharges
Requirements:
  • A minimum of one year of clinical nursing experience, preferably in Hospice or a medical/surgical setting
  • A current RN license in the State of Florida or Alabama, as applicable
  • Reliable transportation, valid driver's license, and current automobile liability insurance
  • CPR certification
