Responsibilities:
  • Supervise and guide field staff to ensure quality of care for patients and caregivers
  • Complete Medicare required documentation
  • Serve as a role model, advocate, and motivator to help nurses acclimate and maintain their competency in nursing
  • Develop and implement learning plans for nursing staff
  • Conduct supervisory visits to observe home interactions and ensure quality indicators are met
  • Ensure documentation is accurate and up-to-date
  • Collaborate with MCPs on learning opportunities and support to staff
  • Participate in performance improvement projects and provide data for QAPI reports
Requirements:
  • Associate's Degree in Nursing or Diploma Certificate in Nursing
  • Minimum of three (3) years of clinical experience in hospice and/or home health
  • NJ State Professional Registered Nurse License
  • AHA Basic Health Care Life Support HCP Certification
  • Certificate for OASIS Specialist - Clinical, Certified OASIS Quality Specialist, or Home Care Clinical Specialist Oasis within 1 year of entering position
