Health Care Liaison

About the Role:
The Health Navigator's role is to simplify the complexities of our healthcare system. You will develop trusted relationships with patients and providers, assisting with various tasks to enable doctors to focus on delivering high-quality care. Navigators work within our partner clinics, meeting with patients, entering data, and coordinating care. Our management and training teams will equip you with the necessary tools and training. Your responsibilities will include:
- Developing strong relationships with patients to support their care
- Meeting with patients during check-in, entering data for reporting and tracking purposes
- Calling patients to schedule appointments or remind them of medication pickups
- Ordering and scheduling procedures, tests, and screenings
- Identifying and tracking patient care gaps
- Maintaining patient interaction records and communicating with providers using electronic health records
- Educating patients on healthcare options, insurance benefits, and common medical conditions
- Assisting patients in accessing community resources
- Coordinating follow-up care after hospital discharge
- Demonstrating compassion, adaptability, attention to detail, patience, and a willingness to collaborate with team members

- You are willing to work a full-time 40-hour week, Monday through Friday, 8am to 5pm, with a one-hour lunch break
- You possess an active, unencumbered driver's license
- You hold a GED or High School Diploma
