Join Our Team as an MDS Coordinator

Bridges Health is seeking a dedicated MDS Coordinator to support our skilled nursing and therapy community. This role involves a direct reporting relationship with our Director of MDS Services, ensuring a seamless flow of information and care.

Position Overview

The ideal candidate will be a detail-oriented licensed nurse responsible for the scheduling and oversight of the Minimum Data Set (MDS) assessments, including Resident Assessment Protocols (RAPs) and individualized Care Plans for residents.

Key Responsibilities

  • Collect and compile data for MDS assessments, ensuring compliance with the RAI manual and HCFA guidelines.
  • Review completed RAPs and all relevant data for accuracy.
  • Maintain meticulous tracking of MDS and Care Plans to guarantee timely completion.
  • Ensure MDS documentation accurately reflects each resident's current medical condition, supported by thorough physical assessments and medical records.
  • Facilitate decisions regarding care plans and document findings on MDS as required by state and federal regulations.
  • Electronically record and transmit MDS data to the state database in accordance with regulatory requirements.

Qualifications

  • Possession of a current and unencumbered Oklahoma Registered Nurse (RN) or Licensed Practical Nurse (LPN) License.
  • Current First Aid and CPR certification.
  • Proficient in basic computer skills, including Microsoft Office.
  • Strong understanding of MDS and RAPs, with the capability to develop comprehensive, individualized care plans.
