Job Summary

As a Hospice Nurse with Ascension, you will provide compassionate, personalized care to patients and their families in a variety of settings. This role requires a strong commitment to delivering high-quality patient care and a passion for working with individuals and families in need.

Responsibilities

• Provide direct nursing care to patients in accordance with established policies, procedures, and protocols of the healthcare organization.
• Implement and monitor patient care plans, serving as a primary coordinator of all disciplines for well-coordinated patient care.
• Note and carry out physician and nursing orders, assessing and coordinating patient's discharge planning needs with members of the healthcare team.

Requirements

• Licensure/Certification/Registration: Licensed Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date required.
• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
• Previous Hospice experience strongly preferred.
