About the Role

We are seeking a skilled Nurse Practitioner to join our team at Agape Care Group, a leading provider of hospice, palliative, and pediatric comfort care services in the Southeast.

Key Responsibilities
  • Provide high-quality, patient-centered care to individuals with life-limiting illnesses and their families.
  • Develop and implement individualized care plans in collaboration with the healthcare team.
  • Conduct thorough assessments, diagnose, and treat patients under the supervision of a physician.
  • Focus on providing palliative care to alleviate symptoms and improve quality of life.
  • Collaborate with the interdisciplinary team to ensure comprehensive care and support.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
Requirements
  • Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing.
  • 2 years of experience within the last 5 years in chronic disease management/long-term care, with hospice background preferred.
  • Reliable transportation and ability to sit, stand, bend, move intermittently, and lift at least 25lbs.
