Physician Primary Care Specialist

We are seeking a highly skilled Physician Primary Care Specialist to join our team at Pacific Medical Centers. As a key member of our healthcare team, you will provide high-quality patient care and manage health issues for a specified patient population.
Key Responsibilities- Provide and manage direct patient care, including physical examinations, evaluations, assessments, diagnoses, and treatment for a specified patient population.
- Engage patients and their families or caregivers in understanding, setting, and monitoring self-management care plans, and document individualized care plans and care coordination in the patient chart.
- Analyze patients' medical history, medication allergies, physical condition, and examination to determine the best care.
- Diagnose medical conditions and prescribe pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.
- Refer patients to specialists or to internal medicine as appropriate.
- Direct and coordinate the patient care activities of nursing and support staff as required.
- Respect and promote patient rights and privacy.
- Respond appropriately to emergency situations.
- Participate in meetings to evaluate practice processes and work flows pertaining to patient care and quality improvement efforts.
- Participate in specified health promotion, education, and/or prevention programs.
- Participate in population health activities as directed by the practice.
- Medical Doctor with 3 to 5 years of direct patient experience, which may include residency.
- Medical License, Board certification, Federal DEA certification.
