Job Summary

Garden Spring Nursing and Rehabilitation Center is seeking a highly skilled Wound Care Nurse to join our team. As a key member of our interdisciplinary team, the successful candidate will be responsible for managing the skin program for the facility, ensuring the highest level of care for our residents.

Key Responsibilities
  • Assess all residents weekly to evaluate the condition of their skin and provide appropriate documentation.
  • Measure all skin areas weekly and document.
  • Initiate and update all care plan information regarding skin.
  • Follow up with physicians to report any resident skin problems and transcribe new orders.
  • Perform skin and related treatments on all residents as assigned, according to physician's orders and professional standards.
  • Communicate with physicians, residents, family members, charge nurses, and other interdisciplinary team members regarding resident status.
  • Represent the Skin Management meeting on a weekly basis and provide necessary information.
  • Monitor and adjust equipment use per facility protocol and resident need.
  • Keep an ongoing record of facility equipment and use.
  • Participate in Quality Improvement activities as assigned.
Requirements
  • RN license preferred.
  • Minimum of 3 years related experience.
