About the Role

We are seeking a skilled Physical Therapist Care Manager to join our team at AdventHealth Home Care West FL. As a Care Manager, you will play a critical role in coordinating and directing the care of our home health patients, ensuring they receive the highest quality of care and support.

Key Responsibilities
  • Coordinate and direct the care of a caseload of home health patients requiring physical therapy as the primary skill.
  • Provide comprehensive assessment, planning, implementation, and evaluation for each patient's care plan.
  • Set priorities for the home care caseload, adapting to the changing needs of patients and families.
  • Optimize schedules daily to support productivity, efficiency, and maintain best practice visit utilization.
  • Perform evaluations, assessing function using objective measures, and document results in the clinical record.
  • Formulate patient-specific plans of care in consultation with patients, families, and physicians.
  • Establish individualized, realistic, measurable patient-centered goals.
  • Periodically reassess patients every 30 days, providing ordered therapy services, functionally reassessing patients, and comparing results to prior assessments.
Requirements
  • Current Physical Therapy License in the State of Practice.
  • Valid Driver's License and current car insurance.
  • BLS certified.
  • Minimum of one-year relevant clinical physical therapy experience.
  • Masters Degree in Physical Therapy.
  • Home Health Case-Manager Certification (preferred).
  • COS-C Certification (preferred).
