Care Navigator

Centers Care Solutions is dedicated to providing high-quality, coordinated healthcare to our members. We are seeking a skilled RN Care Manager to join our team and help us achieve this goal.
In this role, you will work collaboratively with our multidisciplinary team to identify and manage members who require additional support and resources. Your primary responsibilities will include:
- Providing case management through telephonic assessments, care planning, implementation, care coordination, monitoring, and evaluation to ensure members receive the necessary services and supports
- Developing, facilitating, and communicating a plan of care in partnership with members, their families, primary caregivers, physicians, and other providers involved in their care
- Initiating utilization review for preauthorization and post-service reviews
- Utilizing knowledge of community resources and members' benefit structures
- Interacting with providers and facilities in a professional and respectful manner
- Recognizing quality of care issues and escalating them appropriately
- Assisting members with the coordination of services from various settings as needed
- Complying with all CCS standards for documentation
- Participating in case rounds, on-call coverage, and other meetings as requested
To be successful in this role, you must possess an active and unrestricted NYS RN license, strong written and verbal communication skills, and a passion for delivering exceptional patient care. A Bachelor's degree in Nursing is preferred, but not required. Certifications such as CCM are highly valued.
