Kaiser Permanente is seeking a skilled Social Work professional to join our team as a Case Manager Continuing Care Social Worker. This is a part-time/hourly position that requires a strong background in case management, discharge planning, and transfer coordination.
Key Responsibilities- Develop and implement comprehensive care plans for patients with complex needs.
- Coordinate with physicians, staff, and external providers to ensure seamless care transitions.
- Monitor patient outcomes and identify opportunities for cost savings and quality improvement.
- Collaborate with the healthcare team to develop and maintain case management policies and procedures.
- Provide education and support to patients and families to promote self-management and adherence to treatment plans.
- Master's degree in Social Work from an accredited institution.
- Minimum two years of case management experience, preferably with geriatric populations.
- Current and valid LCSW highly preferred.
- Spanish language proficiency preferred.
