Nursing Job Description:
We are looking for a highly motivated and compassionate Registered Nurse to join our team as a Full-Time Home Health RN. In this role, you will work closely with our interdisciplinary team to deliver high-quality patient care in the comfort of patients' own homes.
Key Responsibilities:
- Conduct thorough patient assessments, identifying their physical and emotional needs
- Develop and implement personalized care plans that address patients' medical, social, social, and psychological needs
- Collaborate with healthcare professionals to ensure effective transitions of care, documenting patient progress and outcomes in accordance with regulatory requirements
- Deliver high-quality patient education and support, empowering them to manage their health and well-being
Requirements:
- Current RN licensure in the state of practice
- Current CPR certification
- At least one year of clinical experience as a Registered Nurse (recent LPN experience may be waived)
