Job Title: Licensed Practical Nurse - LPN

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our dynamic care team at Wadsworth Glen Health Care and Rehabilitation Center in Middletown, CT.

About the Role:

As a Licensed Practical Nurse, you will provide direct nursing care to residents under the supervision of the Registered Nurse (RN) and supervise the day-to-day nursing activities performed by the nursing assistant to ensure the highest degree of quality care is maintained at all times.

Responsibilities:
  • Meet the nursing and rehabilitation needs of each resident.
  • Maintain current resident care plans on all residents under your care.
  • Follow physician's orders and render or assist with complete nursing care, treatments, dressings, and other physician orders.
  • Transcribe physician's orders promptly and accurately.
  • Review patient records daily to assure accuracy and completeness.
  • Document comprehensive and complete nursing notes.
  • Visit each resident daily to evaluate resident needs.
  • Maintain accurate awareness of residents on alert list.
  • Document and report any unusual or significant findings; contact the physician when necessary.
  • Administer medication according to procedure.
  • Maintain an accurate and safe control of all drugs for which you are responsible.
  • Initiate any and all emergency procedures as necessary.
  • Demonstrate correct safety techniques.
  • Respect and take care of facility equipment and not wasteful of supplies.
  • Contribute to effective communication and pleasant working conditions.
  • Contribute to and promote positive resident and family relations.
  • Comply with facility Privacy Practices Policies & Procedures, protecting residents' individual health information.
  • Continue growth and expand job knowledge.
  • Demonstrate leadership skills.
  • Other duties assigned by manager.
Requirements:
  • Must be a graduate of an approved LPN/LVN program.
  • Must be CPR certified.
  • Must possess a current, unencumbered license to practice as an LPN/LVN in this state.
  • A minimum, one (1) year experience in a hospital, long-term care facility or other related health care facility is preferable.
