Job Summary
We are seeking a highly skilled Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment Specialist. In this role, you will play a vital part in conducting comprehensive health assessments, reviewing medical history, and providing critical health education to our patients.
Key Responsibilities
- Conduct in-depth health assessments, including reviewing medical history, medication adherence, and social environment, to identify potential health risks.
- Develop and implement personalized health plans, taking into account the unique needs and circumstances of each patient.
- Collaborate with Primary Care Physicians (PCPs) to provide follow-up care, education, and support to patients.
- Work collaboratively with a nationwide network of advanced practice providers to build and maintain relationships and share best practices.

Compensation and Benefits
As a valued member of our team, you can expect a competitive compensation package, including a base rate of $95.00 per completed assessment, with opportunities for additional pay based on your performance and qualifications.
Additionally, you will receive mileage reimbursement for all miles traveled, as well as a no-show payment for 10 or more booked assessments per month.
Requirements
To be successful in this role, you must have:
- A Master's degree in Nursing or a related field, or equivalent experience and licensure.
- Current RN licensure in the state of practice, with prescription authority or the ability to obtain prescriptive authority.
- Board certification by the NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty.
- At least 1 year of experience as a Nurse Practitioner or Physician Assistant.
- Strong computer skills and familiarity with EMR software.
- Excellent verbal and written communication skills.
Travel Requirements
As a Home Risk Assessment Specialist, you may be required to travel to various locations, including homes, skilled nursing facilities, and community environments. You must have a valid state driver's license, be able to drive a car, and have proof of adequate automobile insurance coverage.
