Job Title: RN - Case Management Specialist

INTEGRIS Health Southwest Medical Center is seeking a skilled RN Case Management Specialist to join our team. As a key member of our Case Management team, you will play a vital role in ensuring the best possible outcomes for our patients.

Job Summary:

The RN Case Management Specialist will be responsible for coordinating patient care, managing resources, and facilitating timely discharges. This role requires strong communication and collaboration skills, as well as the ability to work effectively with patients, families, physicians, and other healthcare professionals.

Key Responsibilities:
  • Conduct comprehensive assessments of patients' clinical, psychological, and financial needs
  • Develop and implement individualized care plans to achieve optimal clinical outcomes and financial goals
  • Coordinate with healthcare teams to ensure seamless transitions of care
  • Communicate effectively with patients, families, and healthcare professionals to ensure informed decision-making
  • Monitor and manage patient progress, identifying opportunities for improvement and implementing changes as needed
Requirements:
  • Current licensure as a Registered Nurse (RN) in the State of Oklahoma or current multistate license from a Nurse Licensure Compact (eNLC) member state
  • 5 years of experience in a variety of clinical settings, including home health, inpatient, physician office, and clinic
  • Experience with managed care and payer/provider requirements preferred
  • Excellent interpersonal, communication, and collaboration skills
  • Computer experience required, with proficiency in Windows
