Sutter Health is seeking an experienced RN Case Manager to join our Home Health team. As a Case Manager, you will be responsible for managing patient care services, coordinating with interdisciplinary teams, and ensuring the optimal degree of quality care is maintained.
Responsibilities:
- Plan, organize, and direct patient care services for patients in the assigned caseload
- Coordinate with interdisciplinary teams to implement patient care plans
- Ensure effective management of visit utilization and control of expenses
- May assist in agency-wide orientation, competency evaluations, and consultation to clinical staff
Requirements:
- Graduate of an accredited School of Nursing
- RN-Registered Nurse of California
- BLS-Basic Life Support Healthcare Provider
- DL-Valid Drivers License Class C
- AUTO-Automobile Insurance
Preferred Qualifications:
- 2 years recent relevant experience
- Ability to function independently and as a member of an interdisciplinary team
- Ability to exercise independent, sound nursing judgment in planning and providing patient care
