Providence is seeking a skilled Physical Therapist to join our Home Health team in Walla Walla, WA. As a Physical Therapist, you will play a vital role in developing and implementing therapeutic services to help patients maintain or improve their independence.

Key Responsibilities:

  • Develop and coordinate individualized treatment plans to meet the unique needs of patients
  • Collaborate with an interdisciplinary team to ensure comprehensive care
  • Conduct thorough assessments and evaluations to identify patient needs and goals
  • Implement evidence-based interventions to promote patient progress and independence
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination

Requirements:

  • Bachelor's Degree in Physical Therapy
  • Washington Physical Therapist License upon hire
  • National Provider BLS - American Heart Association upon hire
  • National Provider Identifier (NPI) number within 30 days of hire

Preferred Qualifications:

  • Master's Degree in Physical Therapy or related field
  • 1 year of experience in a therapist capacity in a hospital, skilled nursing facility, or other related medical field
  • Home Health experience
