The RoleAs a Primary Care Physician (PCP) in our team-based care environment, you will be a lead in providing comprehensive health assessments, communicating plans of care with patients and their families, and diagnosing, treating, and managing the Medicare Advantage population with multiple comorbidities. You will also effectively communicate with all members of the care team and facilitate a medically appropriate and effective plan of care to avoid unnecessary duplication of testing and fragmentation of care.
Responsibilities- Robust support allows our PCP to see fewer patients and spend more time with those they do.
- Fully engaged in the concept of "Integrated team-based care" model.
- Focusing on outpatient medicine, continuity of care, health maintenance, and disease prevention.
- Demonstrates both autonomy and proficiency in performance of comprehensive health assessments and communicates Plan of Care with Patients and/or their families.
- Proficient in diagnosing, treating and managing the Medicare Advantage population with multiple comorbidities.
- Effectively communicates with all members of the care team and facilitates a medically appropriate and effective plan of care to avoid unnecessary duplication of testing, and fragmentation of care.

$200,000 - $250,000 per year based on experience, plus bonus opportunities.
Requirements- Graduated from an accredited MD or DO program.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine or Geriatric Medicine.
- Unrestricted Medical License and DEA Licensure in the state of practice or the ability to obtain a license prior to employment.
- Up-to-date CPR Certification and Basic Cardiac Life Support certification.
- Strong organizational and prioritization skills.
- Ability to use Electronic Medical Record (EMR) system - specifically ECW.
- Knowledge of Medical Advantage, HEIDS, MRA and ICD-10 Coding.
- Comfortable managing a primarily Medicare-aged patient population.
- Demonstrated ability to problem-solve complex and multifaceted situations.
