Independent Living Case Manager - Older Youth Services

Salary Range: $43,500 - $48,500 per annum

Are you dedicated to fostering the growth and independence of young individuals? We are seeking a committed Independent Living Case Manager to provide essential support to participants in our Older Youth Services (OYS) program.

The Case Manager will work collaboratively with a dedicated team to:
  • Guide youth in acquiring the necessary skills for a successful transition to self-sufficiency, including access to educational, employment, financial, mental health, emotional, and community resources.
  • Establish meaningful connections with youth through a strengths-based methodology.
  • Engage with various organizations and resources to address the diverse needs of the youth.
  • Respond effectively to youth experiencing crisis situations.
Qualifications:
  • A Bachelor's degree in social work, psychology, sociology, or a related discipline is mandatory.
  • Experience in working with adolescents and young adults is advantageous.
  • Excellent communication abilities and a strong capacity for teamwork are vital.
