Responsibilities
  • Collaborate with the therapy team to provide high-quality care to our residents
  • Conduct thorough evaluations and documentation to inform treatment plans
  • Develop and implement individualized therapy plans to address patient needs
  • Manage workflow and prioritize tasks to ensure efficient treatment delivery
  • Familiarity with PDPM and Medicaid/Medicare billing requirements
Qualifications
  • New Jersey Physical Therapy License
  • Experience in long-term care, specifically Skilled Nursing Facilities
  • Strong organizational, interpersonal, and communication skills
  • Ability to work with diverse patient populations and personalities
  • Creative problem-solving skills
