Position Overview:
Under the supervision of a Registered Nurse and in accordance with medical directives from the Practitioner, the Licensed Practical Nurse (LPN) delivers both direct and indirect nursing services in an outpatient environment.
Key Responsibilities:
Conducts thorough reviews and preparations of medical documentation for assigned patients; initiates follow-up actions for medical records as required; assesses patient needs; identifies and organizes orders for patient requirements for practitioner approval and may accept verbal instructions as per policy; observes, gathers, and records new patient information; communicates with RN or practitioners regarding incomplete data, new observations, or abnormal findings.
Implements fundamental nursing procedures as outlined in the care plan; administers treatments, immunizations, and medications as prescribed by the practitioner; prepares medications for practitioner use; performs authorized point-of-care testing; provides laboratory, x-ray, and other relevant reports to the practitioner for evaluation; informs patients of normal results as directed by the practitioner; considers the unique needs of various age groups when delivering care; prioritizes care based on patient acuity.
Documents the execution of orders/interventions; records findings, instructions, and patient responses on appropriate forms or electronic records in compliance with departmental and/or JCAHO standards.
Monitors patient and family responses to the care plan; reports unexpected reactions to the RN or practitioners; collaborates with RN/Practitioner to adjust the care plan as necessary.
Exhibits and applies proficiency in essential nursing skills and equipment usage; maintains up-to-date knowledge and skills necessary for the role; participates in staff meetings as required; engages in projects as assigned.
Effectively manages emergency patient care situations; adheres to established protocols; notifies the appropriate practitioner/RN.
Delegates basic nursing tasks to unlicensed personnel as appropriate and in accordance with departmental/unit standards; provides clear guidance; acts as a resource for unlicensed staff; follows up on assigned care to ensure compliance with standards, policies, procedures, protocols, and guidelines.
Adheres to discharge plans developed by the Practitioner or RN; processes and/or follows up with referrals as directed by practitioners; provides instructions and pre-printed materials to patients as directed by RN and/or Practitioners, observes and documents patient/family responses to instructions and reports any concerns.
Manages all patient immunization data, including updates to the Hawaii Immunization Registry System (HIR), Electronic Health Records (EHR), and all patient and/or external records during each patient visit.
Prepares any available immunization information prior to the patient’s visit with the provider. Administers and stores immunizations according to Health Plan and VFC guidelines.
Identifies patient immunization needs based on age-specific guidelines per CDC. Completes necessary immunization documentation for record-keeping. Facilitates vaccine inventory control at the assigned clinic, including ordering vaccines and maintaining appropriate stock levels.
Utilizes file maintenance to ensure newly received vaccines are entered into the lot manager system as required. Ensures immunization clinics comply with Health Plan, federal, and state guidelines, including adherence to Health Department site visit protocols. Completes annual VFC enrollment training requirements and maintains all records related to the VFC program as per program guidelines.
Trains new and existing LPNs on emergency vaccine storage protocols.
Acts as a liaison with the state to coordinate VFC (Vaccines for Children) inventory. Generates reports on patients and contacts them for immunization compliance and updates as necessary.
Assists in ensuring adherence to infection control policies and protocols and follows medication administration and storage procedures, as well as Health Plan CDC, State, and federal VFC program guidelines.
Provides educational resources to patients and reviews printed instructions with them; collaborates with RN/Practitioner to modify the teaching plan as needed.
Participates in the orientation of new staff. Demonstrates adequate knowledge, skills, and abilities in the area of specialization. Performs clean/sterile techniques, prepares for procedures, and assists with clinical procedures within departmental scope. Follows the recommended process for handling point-of-use instruments as outlined by the High-Level Disinfection Committee.
Maintains, orders, organizes, rotates, and stocks supplies. Schedules patients for routine care, acute care, and addresses care gaps. Prepares and processes patient documentation.

Preferred Qualifications:
Prior nursing experience.
Previous experience in a clinical setting.
Demonstrated proficiency in word processing and spreadsheet applications.
