We are seeking a skilled RN Case Manager to join our team at Assured Hospice of Moses Lake, a part of LHC Group. As a Hospice RN, you will play a vital role in delivering high-quality end-of-life care to our patients and their families.
Key Responsibilities- Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
- Identify patients' physical, psychosocial, and environmental needs, reassessing as needed every 15 days.
- Develop and coordinate individualized plans of care, maintaining accurate clinical notes on patient condition.
- Collaborate with patients, families, attending physicians, and other healthcare professionals to provide comprehensive care.
- Current RN licensure in the state of practice and one year of clinical experience.
- Current CPR Certification.
- Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
