Job Summary

The General Manager, Health Home Operations is responsible for overseeing the daily operations of the Health Home programs within the Health Home Division. This includes supervising Care Coordination Supervisors and monitoring the quality of service delivery using established standards and workflows.

Key Responsibilities
  • Administer daily operations of Health Home programs and provide supervision to Care Coordination Supervisors.
  • Review and evaluate program protocols and workflows to ensure staff adherence to person-centered and recovery-oriented approaches.
  • Collaborate with the Quality Improvement department and Associate Director to evaluate service delivery effectiveness and identify areas for improvement.
  • Conduct chart reviews and provide feedback to care managers to improve clinical interventions and work performance.
  • Provide weekly supervision and coaching to care management supervisors and ensure all staff receives quality supervision.
  • Monitor quality metrics and ensure staff engagement in daily and weekly encounters to meet departmental benchmarks.
  • Review member charts to monitor staff compliance with assessments, plan of care, HCBS assessments, care conferences, and encounter notes.
  • Consult and participate in crisis interventions as needed.
  • Lead in-service training to improve care management staff knowledge and efficacy on program standards.
  • Meet program goals and objectives in partnership with the leadership team.
  • Participate in planning meetings to report and track program deliverables and outcomes.
  • Conduct meetings with care management staff to provide updates on program performance.
Requirements
  • Masters degree in Mental Health Counseling, Social Work, Creative Art Therapy, Vocational Rehab Counseling, or Nursing with current licensure in NYS.
  • At least two years experience working in behavioral health or clinic environment, with at least three years of care management experience working with a special population, and two years supervisory experience.
  • Strong knowledge and specialty in working with homeless or formerly homeless populations with co-occurring disorders, including HIV, chemical dependency, severe mental health disorders, and other chronic conditions.
  • Knowledge of the Collaborative Care Model and social, behavioral health, substance use, harm reduction, and person in recovery services in New York City.
  • Proven track record in managing and overseeing programs with over 500 members.
  • Excellent written and oral communication skills, with proficiency in Spanish or other languages.
  • Familiarity with electronic health records, Medicaid billings, and regulatory agency web portal systems.
  • Knowledge of regulatory agencies, including NYCDOHMH, NYSDOH, HRA DHS, and OCF, and SAMHSA.

Salary: $70,000-$75,000
