About the Job:
Interim Delaware is seeking a dedicated and compassionate Nurse Practitioner to join our team for home visit roles throughout the State of Delaware.
Key Responsibilities:
- Deliver high-quality patient care in a home-based setting, addressing various medical conditions.
- Conduct regular health assessments, administer medications, and educate patients on self-care strategies.
- Work collaboratively with healthcare teams to develop and implement personalized care plans.
- Maintain open communication with patients, families, and healthcare providers to ensure smooth care transitions.
Required Qualifications:
- Current licensure as a Registered Nurse in the State of Delaware.
- At least six months of experience in a hospital, home care, or nursing home setting within the United States.
- CPR certification.
- Negative TB test result.
- Pass a background check.
- Ability to work independently and manage time effectively.
Compensation: The estimated salary range for this role is $50-$75 per hour, based on experience.
